Adobe Flash plugin necessary

The airway device videos on vam.anest.ufl.edu require the adobe Flash plugin.  
If they do not appear, you can download the most recent Flash version:

Thanks for the help.  Working well.  Love your work on this site.  Airway
lecture given yesterday at OHSU Portland OR cited your work and praised it.
Dr. Ovassapian gave the spotlight lecture.  
 
Have any thoughts on "correct" sizing of ILA or fastrac LMA for blind intubation with these devices?

The replies below are from 2 attendings at the University of Florida in response to the above query about sizing:
 
Attending 1:
Sem, I dont have a sizing algorithm except to say that the devices don't always line up perfectly and thus it's helpful to lead with a scope vs just doing it blindly as then you can manipulate the device to line up with the laryngeal inlet.

This is a great question and there is no good discussion of this question elsewhere that I am aware of. Clinically, I will say that I have encountered several significant problems with the ILAs or just intubating through a regular LMA.  
 
1. It can be difficult to slide the ETT in, and conversely, to extricate the ILA once the ETT has been advanced,  
 
2. To get the ETT to advance through the larynx.
 
The clinical solution to 1 depends on how you are intubating. If you are using the Fastrach or any LMA device with a soft tube (the LMA-brand ett or a Sheridan reinforced ett), one is best served to use the LMA size that best fits the ETT size and be very generous with the external lubrication. Using too large an lma, with a larger shaft diameter, allows the soft etts to 'z' in the shaft, essentially braking the tube and complicating advancement. If this happens, one can either 'screw' the soft ett forward or switch to a conventional ett that is stiffer.  
 
If the soft tubes are not used, and a conventional tube is, using the largest size ILA that will fit through the mouth is advantageous for several reasons. The larger LMA gives additional caliber to the intubating channel and tends to center better and stay seated better in the hypopharynx. And with lubrication, decreases sticking both during advancement and ILA removal. It is wise, in my opinion, to lubricate the entire lma channel with the ett before starting; this both confirms the ett fits through the chosen lma, lets the operator feel the resistance, and distributes the lubricant over both the lma and ett surfaces.
 
My personal preference is to always use a self-centering tube (Parker - which is stiff, LMA ETT - which is soft) and/or a bronchoscope. With a stiff tube I use a large LMA, with a soft tube the smallest LMA that allows advancement of the ett. If a fiberoptic scope is available, using it can help reduce the chances of getting the ett bevel caught at the glottic opening. Obviously, this part improves with experience and the application of a little finesse so one does not apply excessive force (presumably to overcome the friction on the ILA but may be on the VC) during advancement of the ett.
